POSITION SUMMARY
  
The Emergency Department (ED) Patient Guide assists non-urgent patients access to the highest quality and most affordable care appropriate to their needs. Improves the patient experience and health outcomes through patient education and directing appropriate resource coordination with community care coordinators, case managers, or patient guides. Collaborates with the nursing ED case managers to facilitate referrals to a more appropriate care setting and follows up with patients to discuss health status and experience with new primary care appointments.
  
This role also makes sure educational and discharge planning needs of patients are met, including coordinating with formal education programs such as diabetes, and the community education and resource center. They will contact patients after discharge from home care and collect information on adjustment, functional status, quality of life and satisfaction measures.
  
They will also work to identify certain patient populations including:

* At risk patient groups for readmission and facilitate placements of patients to the next level of care in the continuum. They will use the PLACE tool in Allscripts with the goal of reducing readmission through coordination of community services.
* Dual eligible patients with Medicare and Medicaid
* High utilizers who are coming in repeatedly for low-acuity issues. They will then identify the issue/barrier and work with patient and/or community partners to reduce barriers.
  
QUALIFICATIONS
  
Education

* Associates degree or the equivalent in education and experience
* Bachelors degree or additional advanced education, preferred
  
Experience/Abilities

* 3-5 years of customer service experience including at least one year in a healthcare setting
* Able to problem solve, interpret, and deal with variables by using critical thinking skills. 
* Able to prioritize a number of simultaneous requests and tasks. 
* Most possess excellent interpersonal and communication skills. 
* Adaptability, flexibility, and a positive non-judgmental attitude are a must.
